HOW DO I COMPLETE THE AFFIDAVIT OF SUPPORT FORM?

To download a copy of the OSU Affidavit of Support Form go to: http://www-afa.adm.ohio-state.edu/apps/pdfs/ig-affad.pdf

After completing the form, please fax it to the attention of "Financial Review", at: (614)688-3593.

Please Note: The Affidavit of Support form and the accompanying certified bank statement must be in the same person’s name. If you have multiple sponsors, each sponsor needs to provide a set of financial documents in their name; i.e., whoever provides a bank statement must also sign and date the Affidavit of Support form. In completing the section entitled "Sponsorship" on the Affidavit, it is necessary to insert the amount of support according to the amount of years you will be enrolled in your degree program (The time frame to complete a Master's degree program is 2 years and a PHD program is 3 years at Ohio State). However, your certified bank statement and/or sponsorship letter only needs to document one year of funding.

Additional clarifications include:
-The date of financial support documents must be one year or less at the time of financial review.
-The available funding must be liquid; i.e., property titles, statements from chartered accountants, or salary statements are not accepted documentation.
-A Government or Sponsoring Agency (i.e., organization, institution, or employer) must provide the following information on a sponsorship letter:
• Are they paying all tuition and fees? How much?
• Are they covering your living expenses? How much?
• How long is your sponsorship valid?
• Will the sponsor be willing to provide extra support for dependent(s) or English preparation courses, if applicable? If so,how much?
