This application checklist is for all graduate programs except Chemistry, Dentistry, Doctor of Physical Therapy, Physics, Public Health (excluding PEP and Health Services Management and Policy), Speech Language Pathology and all graduate Business programs.

1. Apply online at gradapply.osu.edu with the nonrefundable application fee ($40).
2. One original copy of transcripts or record of marks for each university-level school attended. Include English translation of each of any foreign documents.
3. Official GRE or GMAT scores, if required.
4. Other test scores required by your intended program.

1. A statement of intent, unless the program specifies a different requirement for this item.
2. One original copy of transcripts or record of marks for each university-level school attended. Include English translation of each of any foreign documents.
3. Three letters of recommendation written on recommenders' original letterhead
stationery, accompanied by completed Ohio State Reference

4. Curriculum vitae/resume of no more than two pages for all applicants who wish to be considered for a fellowship.
